Femme de 46 ans, droitière, chez qui a été découvert il y a 10 ans de façon fortuite deux méningiomes de l’angle ponto-cérebelleux gauche. Aucun suivi radiologique n’a été pratiqué. La patiente a reconsulté récemment pour des troubles de l’équilibre importants avec une audition gauche quasi-normale.
L’IRM cérébrale pondérée en séquence T1 avec injection de gadolinium montre un effet de masse sur le tronc cérébral par 2 méningiomes qui strangulent le paquet acoustico-facial. Il n’y a pas d’œdème du tronc cérébral ni du cervelet sur les séquences IRM T2.

La patiente va être opérée par une voie de la pointe du rocher gauche, c’est à dire extradurale sous-temporale trans-tentorielle sous monitoring électrique permanent du nerf facial.
